Job Summary
Senior Case Manager/Case Manager position at Sabra Goldman House in Queens Village, NY. Responsibilities include conducting home visits, obtaining psychosocial histories, coordinating with psychiatric care, ensuring access to psychiatric, medical, and entitlement services, monitoring medications and side-effects, administering health screening tools, developing individualized Service/Support Plans, delivering community rehabilitation and housing support services, documenting client progress, training clients in safety procedures, preparing residents for independent living, and crisis intervention. Requires an M.A. in psychology or related field (or B.A. with 3+ years direct care experience); strong communication skills; experience with adults with serious mental illness preferred. Salary ranges $52,081–$60,355 plus benefits; schedule Tuesday through Saturday, 11:00am–7:00pm. Located in Queens Village, NY (US). ACMH is an equal opportunity employer.
Required Qualifications
- M.A. in psychology or a human service related field
- B.A. in psychology or a human service related field with a minimum of 3 years of direct care experience
- experience working with adults with serious mental illness preferred
- strong written and verbal communication skills
