Job Summary
Senior Case Manager/Case Manager provides case coordination and delivers community rehabilitation services in the Bronx. Responsibilities include conducting home visits, obtaining psychosocial histories, liaising with psychiatric care providers, ensuring access to psychiatric, medical, and entitlement services, monitoring medications and side-effects, administering health screenings, developing individualized rehabilitation plans, delivering a range of recovery and housing-support services, assisting with education/employment and entitlement needs, documenting client progress, training clients in safety procedures, preparing residents for independent living, facilitating crisis intervention, and serving as Qualified Mental Health Staff Person. Requires MA in psychology or related field (or BA with 3+ years direct care experience); strong communication skills; Master’s Degree preferred.
Required Qualifications
- M.A. in psychology or a human service related field
- B.A. in psychology or a human service related field with a minimum of 3 years of direct-care experience
- experience working with adults with serious mental illness preferred
- strong written and verbal communication skills
- Master’s Degree preferred
