Job Summary
Conduct home visits to obtain psychosocial histories, administer health screenings, and monitor client medication adherence. Develop individualized rehabilitation plans and deliver community services including assertiveness training, daily living skills, and substance abuse support. Facilitate crisis intervention, staff emergency phones on rotation, and oversee apartment maintenance. Document client progress, update service plans, and prepare residents for independent living transitions.
Required Qualifications
- M.A. in psychology or a human service related field
- B.A. in psychology or a human service related field with a minimum of 3 years of experience providing direct care in human services
- Strong written and verbal communication skills
- Master's Degree preferred
Desired Qualifications
- Experience working with adults with serious mental illness
