Job Summary
Senior Case Manager provides case coordination and delivers community rehabilitation services. Responsibilities include conducting home visits, obtaining psychosocial histories, assessing functional skills, liaising with psychiatric care providers, ensuring access to psychiatric/medical/services and entitlements, monitoring medication adherence, administering health screening tools, developing individualized Service/Support Plans, delivering housing and education/employment support, health care and crisis/continuation services, documenting client progress, training clients in safety procedures, preparing residents for independent living, and providing coverage for program staff. Requires MA in psychology or related field (Master’s Degree preferred), BA with relevant experience, and strong written/verbal communication skills. Posts include supervising and coordinating rehabilitation, crisis intervention, and compliance with agency procedures; location in Manhattan (Harlem).
Required Qualifications
- M.A. in psychology or a human service related field
- B.A. in psychology or a human service related field with a minimum of 3 years of direct care experience in human-services
- experience with adults with serious mental illness preferred
- strong written and verbal communication skills
- Master’s Degree preferred
