Conduct outreach work with care-avoiding clients in their homes, streets, and community locations to build trust and reconnect them with mental health services. Map psychiatric and psychosocial problems through active contact and supportive conversations, while collaborating intensively with psychiatrists, psychologists, and social partners. Manage crisis situations independently and assist clients in formulating help requests within a multidisciplinary team. This role offers professional autonomy and a mix of psychiatric and social domain challenges, with a salary range of €3,560 to €5,294 gross for a 32-36 hour contract.
