Job Summary
Provide spiritual support and counseling to patients, families, and caregivers; assess spiritual needs and help incorporate them into the patient’s plan of care; offer comfort, guidance, and emotional support during illness, decline, grief, and end-of-life care; collaborate with nurses, social workers, aides, and other members of the interdisciplinary team; participate in care planning and team meetings; travel within the service area to serve patients in Canonsburg, PA; maintain comprehensive documentation of care and encounters.
Required Qualifications
- Experience in spiritual care, pastoral care, ministry, chaplaincy, hospice, home health, or a related setting
- A compassionate and patient-centered approach
- The ability to work independently while collaborating with an interdisciplinary team
- A valid driver’s license and reliable transportation for travel within the service area
- Strong communication and documentation skills
