Spiritual Care Coordinator: Home Health and Hospice

Provide spiritual care, prayer, counseling, and emotional support to patients, families, and caregivers while assessing spiritual needs and incorporating them into each patient's plan of care. Offer comfort through presence, conversation, scripture, or prayer while collaborating with nurses, social workers, aides, therapists, and physicians on interdisciplinary care planning meetings and documenting visits. Lead or assist with memorial services, bedside prayers, and family support as appropriate.
