Spiritual Care Coordinator: Home Health and Hospice

Provide spiritual care, prayer, counseling, and emotional support to patients, families, and caregivers while assessing spiritual needs and incorporating them into each patient's plan of care. Offer comfort through presence, conversation, scripture, or prayer while collaborating with nurses, social workers, aides, therapists, physicians, and other members of the interdisciplinary care team. Participate in care planning meetings, document spiritual care visits, and lead or assist with memorial services, bedside prayers, and family support as appropriate. Maintain a valid driver's license for travel throughout the service area in Washington and Greene counties.
