Job Summary
Develop an individualized speech therapy plan of care and adjust it as patient needs change. Guide patients through therapeutic and self-care activities to improve swallowing, breathing, cognition, speech, and communication. Educate patients and their families on disease management and preventative measures while documenting progress per agency protocol and Medicare guidelines. Observe and report patient reactions and changes to the attending physician. Provide home-based skilled care in New Vienna, Pleasant Plain, and surrounding areas Monday through Friday from 8:15 AM to 5:00 PM with no on-call responsibilities.
Required Qualifications
- Speech Therapist with a current license in the state of employment
- Minimum one-year experience as a ST in an acute care setting
- Valid driver's license and auto insurance in your name as a driver
- Capable of all physical demands
