Job Summary
Evaluate residents with speech, language, cognitive, communication, and swallowing disorders while developing and implementing individualized treatment plans. Provide skilled therapy services, monitor resident progress, and collaborate with physicians, nursing staff, and families to support positive outcomes. Educate residents, families, and caregivers on treatment strategies and maintain accurate documentation, assessments, care plans, and progress notes. Participate in interdisciplinary meetings and discharge planning activities while ensuring compliance with all regulatory, clinical, and facility standards. Required Qualifications
- Master's Degree in Speech-Language Pathology
- Active New York State Speech Therapist/Speech-Language Pathologist license
