Job Summary
Conduct comprehensive in-home speech-language pathology evaluations and deliver skilled visits for patients with aphasia, cognitive-communication impairments, and dysphagia. Build and execute clear plans of care with measurable functional goals while driving multidisciplinary coordination and coaching patients or caregivers on practical home programs. Own patient outcomes by delivering low avoidable-hospitalization rates and strong experience scores through active participation in case conferences and team huddles. Maintain a high-quality caseload of at least 10 productivity points weekly, ensuring all documentation is defensible, efficient, and consistent. Operate as a referral-source ambassador to build trust with discharge planners, ALFs, and physicians in the Detroit market.
Required Qualifications
- Active Michigan ST license (in good standing)
- Reliable transportation
- Valid driver's license
- Auto insurance
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health PTA scope
- Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
- Strengthening, functional mobility, and home safety assessment/modification education
- Patient/caregiver coaching with practical home programs
