Job Summary
Home Health Speech-Language Pathologist (SLP) providing 1:1 patient care in Crestview, FL and surrounding areas. Responsibilities include conducting speech, language, cognitive, and swallowing evaluations; developing individualized treatment plans based on physician orders; providing therapy for communication, cognition, and dysphagia; educating patients and caregivers to improve functional outcomes; collaborating with physicians and interdisciplinary teams; completing OASIS documentation as required; and documenting visits using AI-assisted technology. Flexible schedule with PRN or 1099 contractor arrangement, mileage reimbursement, and opportunities for career development. AI-assisted charting, user-friendly EMR (WellSky/Kinnser), structured onboarding, and ongoing training support professional growth. Employment contingent on AHCA Level 2 background screening.
Required Qualifications
- Active Florida Speech-Language Pathologist license
- Valid driver’s license with reliable transportation
- Home health experience preferred but not required
- OASIS/Medicare documentation experience preferred
