Job Summary
Speech Language Pathologist (SLP) delivering skilled, home-based therapy in patients' homes in San Antonio, TX. Provides speech therapy services per physician orders and individualized care plans, conducts evaluations of communication and swallowing needs, develops and adjusts treatment plans, educates patients, families, and caregivers on home-based strategies, completes timely documentation including OASIS assessments, and collaborates with physicians and interdisciplinary teams to ensure high-quality care. Requires Master;s or Doctoral degree in SLP, Texas licensure (or eligibility), ability to travel within the San Antonio area, and strong communication skills; operates with autonomy in a home-health setting and participates in a collaborative care team.
Required Qualifications
- Masters or Doctoral degree in Speech Language Pathology
- Current SLP license in the state of Texas (or eligibility)
- Strong communication skills and the ability to connect with diverse patients and families
- Adaptability in dynamic, home-based care environments
- Organized, self-directed approach to managing schedule and documentation
- Willingness to travel between patient homes throughout the San Antonio area
