Job Summary
Home Health Speech Language Pathologist (SLP) leads evaluation and treatment of patients with speech, language, cognitive, and swallowing disorders in a one-on-one home-based setting. Conduct comprehensive evaluations, develop and implement individualized plans of care to improve communication, cognition, and safety, provide therapeutic interventions using evidence-based techniques and specialized tools, assess the home environment and recommend adaptive equipment or communication aids, educate patients and caregivers on exercises, compensatory strategies, and safety techniques, monitor progress and adjust treatment plans, communicate proactively with physicians and care team, document all care in the EMR, and collaborate with nurses, therapists, and interdisciplinary teams for coordinated care. Requires Master’s or Doctoral degree in Speech-Language Pathology, current state SLP license, minimum 1 year clinical experience (home health preferred), CCC-SLP preferred, strong clinical judgment, excellent communication skills, willingness to travel between patient homes, CPR certification, and valid driver’s license with reliable transportation and auto insurance.
Required Qualifications
- Master’s or Doctoral degree in Speech-Language Pathology
- Current SLP license in the state of practice
- Minimum of 1 year clinical experience (home health preferred)
- Certificate of Clinical Competence (CCC-SLP) preferred
- Strong clinical judgment and ability to work independently
- Excellent communication and patient education skills
- Comfortable in a field-based role with travel between patient homes
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
