Job Summary
Conduct comprehensive evaluations of speech, language, cognitive, and swallowing function to develop individualized plans of care. Provide therapeutic interventions using evidence-based techniques, assess home environments for adaptive equipment, and educate patients and caregivers on safety strategies. Monitor patient progress, adjust treatment plans, and communicate proactively with physicians and interdisciplinary teams. Document all care accurately within the EMR and participate in case conferences and discharge planning. Requires a Master's or Doctoral degree, current SLP license, and minimum one year clinical experience. Full-time home health position with travel between patient homes.
Required Qualifications
- Master's or Doctoral degree in Speech-Language Pathology
- Current SLP license in the state of practice
- Minimum of 1 year clinical experience
- Strong clinical judgment and ability to work independently
- Excellent communication and patient education skills
- Comfortable in a field-based role with travel between patient homes
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Auto insurance
Desired Qualifications
- Home health or community-based care experience
- Certificate of Clinical Competence (CCC-SLP)
- Confidence managing swallowing (dysphagia) and cognitive-communication disorders
- Ability to adapt treatment plans to real-life home environments
- A balance of clinical expertise, compassion, and accountability
