Job Summary
Speech Therapist (SLP) providing ongoing therapy to home health clients in the Montrose-Delta area per physician orders. Evaluates and treats clients with neurological-origin disorders across language comprehension and expression, cognition, articulation, voice, fluency, and swallowing. Maintains timely documentation and communicates progress with families, physicians, and other health team members; formulates effective treatment plans and obtains physician approval for services; complies with state Speech Language Pathology Practice Act. Requires Master’s degree in Speech Pathology (CCC or equivalent), state licensure eligibility, and independent functioning within the department. The role supports geriatric rehabilitation within a Medicare/Medicaid-certified home health setting.
Required Qualifications
- Master’s degree in Speech Pathology with Certificate of Clinical Competence (CCC) or equivalent experience
- Valid license in Speech Pathology in the state
- Registration with the Department of Health or eligible
- Recommended Member of Speech Language Hearing Association
- Experience with adults/children with disorders of neurological origin including language, cognition, articulation, voice, fluency, swallowing
- Ability to function independently within the department
