Job Summary
Assess and provide services to Schofield Residence patients requiring speech, language, and swallowing treatments. Perform communication evaluations and conduct individual therapy; prepare initial evaluation, progress notes, recertification, and discharge reports; counsel patient and caregiver; participate in patient-centered conferences; maintain equipment and therapy materials; participate in and/or present inservice programs; maintain accurate clinical records and follow Medicare/Medicaid guidelines and NYS Department of Health rules; ensure compliance with regulatory guidelines. Requires Master’s degree in Speech/Language Pathology, NYS licensure, and ASHA CCC-SLP.
Required Qualifications
- Master’s degree in Speech/Language Pathology
- Licensure and current registration to practice as a Speech Therapist in NYS
- Certification of Clinical Competence (CCC) from the American Speech-Language-Hearing Association
