Evaluate patients' functional levels and progress toward goals, then implement individualized treatment plans in accordance with physician orders. Supervise Speech Therapy Assistants, document treatments and care plan updates accurately, and assist patients in developing home programs. Recommend adaptive equipment and evaluate home settings as needed. Collaborate with healthcare providers to ensure coordinated care and complete discharge summaries at case completion.
