Job Summary
PRN Speech-Language Pathologist to assess and treat residents with communication, cognitive, and swallowing disorders in a skilled nursing facility; develop and implement personalized therapy plans; provide interventions including compensatory techniques, exercises, and alternative communication strategies; educate residents, families, and staff on communication and swallowing management; collaborate with interdisciplinary teams to ensure coordinated and effective care; monitor residents’ progress, adjust treatment plans as necessary, and document outcomes; maintain accurate EMR documentation in compliance with regulatory and facility guidelines; participate in quality improvement initiatives to support the rehabilitation department; on-site in Florence, CO.
Required Qualifications
- Master’s degree in Speech-Language Pathology from an accredited program
- Current and valid Speech-Language Pathologist license in the state of Colorado
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred
