Speech Therapist- Independent Contractor

Speech Therapist- Independent Contractor provides in-home speech therapy services on an intermittent basis under physician orders, at a rate of $60.00 per hour. Responsibilities include initial and ongoing assessments (including OASIS), developing and implementing an interdisciplinary care plan for speech therapy, interpreting therapy services to clients and families, ensuring appropriateness of physician orders, preparing clinical and progress notes, arranging outpatient services when needed, participating in discharge planning, and supervising the Home Health Aide biweekly when speech therapy is the sole professional discipline involved in the client's care. Coordinate with physicians and team members, and document patient status and therapy plan of treatment.
