Speech Therapist, Home Visits

Evaluate, direct, and provide speech/language pathology services to patients in the home or facility. Administer diagnostic tests and therapeutic treatments, including audio logic screening, while assessing disorders of speech, voice, language, hearing, and swallowing. Participate in developing and reviewing the Plan of Treatment and Plan of Care, observe patient responses, and submit documentation, reports, and care plans as required. Provide instruction to patients and families on alternative communication systems and counsel healthcare staff. Maintain confidentiality and engage in care coordination and discharge planning. This part-time role (20-24 hours/week) supports CenterWell Home Health's mission to help patients regain independence and quality of life through personalized rehabilitation.
