Speech Therapist - Home Health

Conduct comprehensive assessments using the OASIS data set to evaluate speech, language, hearing, and oral myofunctional disorders. Provide rehabilitative services under physician orders, coordinate care planning, and instruct caregivers on correcting speech disabilities. Act as Case Manager for therapy-only cases, participate in staff education and patient case conferences, and maintain strict confidentiality. Record clinical findings, report treatment reactions and condition changes to the physician, and submit required documents timely. Maintain quarterly goals established in the annual performance evaluation. Requires a valid Florida Speech Pathologist license and approved curriculum.
