Job Summary
Provide speech-language pathology services in home health/home care settings. Evaluate, direct and provide speech/language pathology services to patients in the home or facility; participate in the development and periodic review of the Plan of Treatment and Plan of Care; assess and treat disorders of speech, voice, language, hearing and swallowing; administer and interpret diagnostic tests and therapeutic treatments; observe and report changes in patient condition; instruct patients in use of alternative communication systems; counsel patients, families and healthcare staff; maintain and submit documentation and reports; participate in care coordination and discharge planning; maintain confidentiality; attend internal staff development and continuing education; requires ASHA CCC in Speech Language Pathology or Audiology, minimum six months experience, current license, CPR, and valid driver’s license with transportation.
Required Qualifications
- Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
- Minimum of six months experience as a speech therapist/slp
- Current and unrestricted license
- Current CPR certification
- A valid driver’s license, auto insurance, and reliable transportation are required.
