Job Summary
Conduct comprehensive speech, language, cognitive, and swallowing assessments in patients' homes and develop individualized treatment plans based on physician orders. Provide therapeutic interventions for conditions including stroke, brain injury, dementia, and Parkinson's disease while educating families on home exercise programs. Document progress and maintain accurate clinical records in compliance with Medicare and insurance requirements, collaborating with physicians and nurses. Requires Master's degree, current licensure, and CCC-SLP; valid driver's license and CPR certification mandatory. Full-time or part-time roles with flexible Monday through Friday scheduling and a typical caseload of 5-7 patients daily.
Required Qualifications
- Master's degree in Speech-Language Pathology from an accredited program
- Current state licensure as a Speech-Language Pathologist
- Valid driver's license

- Current CPR certification
Desired Qualifications
- Certificate of Clinical Competence (CCC-SLP) from ASHA
- Previous experience in home health, skilled nursing, or rehabilitation setting
- Strong clinical assessment and treatment skills across the lifespan
- Experience with dysphagia evaluation and treatment
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Compassionate approach to patient care
- Proficiency with electronic documentation systems
