Job Summary
Conduct comprehensive assessments and develop individualized Plans of Care for patients receiving home health services, collaborating with physicians and interdisciplinary team members. Provide evidence-based therapeutic interventions to improve speech, language, cognition, and swallowing functions while continuously evaluating progress and adjusting care plans. Document care accurately to ensure compliance with agency and regulatory standards, and foster communication with patients, families, and care teams. Participate in interdisciplinary case conferences and quality initiatives while adhering to accreditation standards. This role involves regular travel to patient homes and requires lifting up to 50 pounds and responding quickly in emergencies.
Required Qualifications
- Master's degree in Speech-Language Pathology from an accredited institution
- Active SLP license in the state of practice in good standing
- Current CPR certification
- Valid driver's license with reliable transportation
- Knowledge of state practice acts, ACHC standards, and home health documentation requirements
- Strong communication skills
- Clinical judgment
- Documentation skills
- Must follow universal precautions
- Infection control procedures
- Personal protective equipment (PPE) requirements
Desired Qualifications
- Minimum one year of experience in a home health or medical setting
