Job Summary
Evaluate assigned patients through formal and informal diagnostic assessments to determine levels of functioning and intervention needs. Develop and implement individualized treatment programs, establishing goals and strategies based on ongoing evaluations across speech, language, cognition, voice, and dysphagia deficits. Provide direct therapy services in various functional settings while coordinating with rehabilitation, medical, and nursing care plans. Prepare written documentation including evaluation results, treatment plans, and progress notes. Communicate effectively with patients, families, physicians, and interdisciplinary team members regarding status and needs. Participate in department program development and monitor clinical internships with affiliated students.
Required Qualifications
- CCC-SLP - American Speech-Language-Hearing Association (ASHA) or presently active in a supervised clinical fellowship year and certification received within one year of employment Upon Hire
- Speech-Language Pathologist - Licensing Board
- Active license in the state of practice Upon Hire
- Visual acuity adequate to perform position duties
- Ability to communicate effectively with others
- Ability to hear, understand and distinguish speech and other sounds
- Ability to work the hours specified
- 1-3 years Speech Language Pathologist in a hospital, SNF, outpatient and/or home health setting
