Job Summary
Evaluates patients and develops treatment plans and provides direct speech therapy services addressing deficits in speech, language, cognition, voice, and dysphagia in the home health setting; collaborates with rehab, medical, and nursing teams; documents evaluation results and progress; maintains age-specific care from infant to geriatrics; requires CCC-SLP or state license with active ASHA certification or supervised fellowship plus licensing; 1-3 years preferred; sign-on bonus up to $10,000 and potential student loan repayment; participates in interdisciplinary program development and reports to and coordinates with family, physician, and team; supports CARE values and safety compliance.
Required Qualifications
- CCC-SLP - American Speech-Language-Hearing Association (ASHA) or presently active in a supervised clinical fellowship year and certification received within one year of employment Upon Hire
- Speech-Language Pathologist - Licensing Board An active license in the state of practice Upon Hire
