Job Summary
Provides speech therapy services to patients with speech, language or swallowing disorders in home health and hospice settings. Responsibilities include assessing and documenting speech therapy data related to patient assessments, interventions and outcomes; identifying changes in physical and emotional parameters; reviewing treatment plans and modifying or writing new programs; maintaining documentation as required by law and policy; instructing patients on ways to monitor speech and practice new skills; and performing other job-related duties as assigned. Requires a Master’s degree (or foreign equivalent), state licensure as a Speech Pathologist, CPR/BLS certification, and a valid driver’s license (must be 21+). ASHA CCC is preferred. Vaccination compliance per organizational policy and E-Verify participation are noted as organizational requirements.
