Job Summary
Evaluate assigned patients through formal and informal diagnostic assessments to determine levels of functioning and intervention needs across speech, language, cognition, voice, and dysphagia. Develop and implement individualized treatment programs while establishing goals based on ongoing evaluations. Provide direct therapy services in home settings, coordinating with rehabilitation, medical, and nursing care plans to address deficits in infants through geriatrics. Prepare written documentation including evaluation results, treatment plans, and progress notes. Communicate effectively with patients, families, physicians, and interdisciplinary team members to monitor patient status and needs. Participate in department program development and evaluate clinical internships with students.
Required Qualifications
- CCC-SLP - American Speech-Language-Hearing Association (ASHA) or presently active in a supervised clinical fellowship year and certification received within one year of employment Upon Hire
- Speech-Language Pathologist - Licensing Board An active license in the state of practice Upon Hire
- visual acuity adequate to perform position duties
- ability to communicate effectively with others
- hear, understand and distinguish speech and other sounds
- 1-3 years Speech Language Pathologist in a hospital, SNF, outpatient and/or home health setting
