Job Summary
Conduct client assessments and manage a discipline-specific caseload using a case management approach. Develop, implement, and evaluate tailored therapy programs aligned to client goals across clinic, home, school, and community settings. Collaborate with allied health professionals to achieve optimal outcomes while delivering specialized programs such as group therapy and parent workshops. Record all treatment notes in PMS within 24 hours and provide timely reports to stakeholders. Maintain strict adherence to privacy, confidentiality, and current best practices while participating in team meetings and professional development opportunities.
Required Qualifications
- Demonstrated experience in solution-focused therapy and early intervention
- Understanding and supporting the principles of individualised services including the ability to adapt services to meet individual needs
- Demonstrated ability to engage with a range of children and families and external stakeholders
- Strong communication and organisational skills
- Exceptional observational and report writing skills
- Ability to work within a multidisciplinary team and the ability to work autonomously in the community
- Relevant qualification in Speech Pathology
- Registration with AHPRA
- Registration with SPA
- NDIS Workers Screening Check
- Current Drivers' Licence and vehicle
- Current Working with Children Check or Working with Vulnerable People Registration (state specific)
- National Police Check
- Accredited First Aid Certificate
