Job Summary
Conduct thorough speech-language evaluations and assess home environments to support therapy goals for patients with speech, language, voice, and swallowing disorders. Develop and implement personalized plans of care, providing specialized therapy using tailored techniques and equipment. Coordinate care with referring physicians and participate in case conferences, training sessions, and interdisciplinary meetings to ensure cohesive delivery. Accurately document evaluations, treatment plans, and progress in accordance with agency policies while educating patients, caregivers, and family members on therapy programs. Supervise home health aides when services are directly related to the speech-language plan of care and make appropriate referrals for additional patient needs.
Required Qualifications
- Master's degree in Speech-Language Pathology from an accredited program
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA
- State license to practice as a Speech-Language Pathologist
- At least one year of clinical experience in speech-language pathology
- Current BLS (CPR) certification – hands-on training required
- Excellent communication, documentation, and organizational skills
