Job Summary
Provide one-on-one Speech Language Pathology to older adults in-home or within senior living communities, developing and delivering individualized plans of care, and delivering evidence-based therapy. Role supports in-home and senior living settings with optional telehealth/virtual care to extend access and flexibility. Responsibilities include evaluations, treatment sessions, documentation in FOX’s EMR, collaboration with an interdisciplinary team, and patient/caregiver education to promote safety, function, and aging in place. Requires SLP license in state of practice (or eligibility), CCC, CPR, EMR proficiency, and ability to drive to patient homes and facilities. Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Current CPR certification
- Current Certificate of Clinical Competence (ASHA CCC)
- EMR proficiency
- Driving to patient homes and senior living communities
- Ability to meet physical demands of in-home and community-based care
- Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Wearing PPE when required
- Ability to communicate and educate patients, caregivers, and staff effectively
- Telehealth / virtual care experience or willingness to incorporate into schedule
