Job Summary
Provide one-on-one Speech Language Pathology to older adults in private residences, senior living communities, and memory care settings. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment focused on mobility, independence, and functional longevity. Document sessions effectively within FOX's EMR system and collaborate with interdisciplinary teams including PTs, OTs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging in place, while incorporating telehealth or virtual care when appropriate to extend access. Create and manage your own treatment schedule with flexibility to adjust hours and visit types.
Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Current CPR certification
- Current Certificate of Clinical Competence (ASHA CCC)
- Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Ability to perform CPR
- Ability to provide moderate to maximum assistance with patient transfers and bed mobility
- Ability to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Ability to drive to patient homes and senior living communities as part of the Geriatric House Calls model
- Ability to wear PPE when required
