Job Summary
Conduct comprehensive assessments of patients to determine needs and develop treatment plans, then deliver personalized rehabilitation therapy in homes or care facilities. Constantly evaluate evolving patient needs and adjust plans of care accordingly. Practice independently within the community as part of an interdisciplinary care team. This role supports VNS Health's 130-year history of providing home- and community-based care to New Yorkers.
Required Qualifications
- Minimum of one-year speech language pathology experience
- Successful completion of probationary period as a VNS Health Rehab Intern
- Licensure to practice as a Speech Language Pathologist/Audiologist in New York State
- Valid driver's license
