Job Summary
Conduct patient evaluations, assess home environments, and identify communication and swallowing needs. Establish measurable goals, administer evidence-based therapeutic interventions, and recommend assistive devices. Provide palliative-focused speech therapy for Hospice patients, enhancing quality of life through communication and swallowing support. Instruct patients, families, and team members in treatment regimens. Document evaluations, clinical notes, and progress timely, meeting Medicare CoPs, PA licensure standards, and agency policies. Collaborate with physicians, interdisciplinary teams, patients, and caregivers to coordinate comprehensive care. Maintain professional competence through continuing education and best-practice standards. Work Monday through Friday from 8:00 to 4:30.
Required Qualifications
- Graduate of an accredited Speech Therapy program
- Current SpeechTherapist license in the state of practice
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Current automobile insurance
- Strong communication skills
- Strong organizational skills
- Strong clinical assessment skills
- Frequent travel within assigned territory
- Ability to safely assist patients with transfers and mobility activities
- Ability to stand, walk, bend, reach, and lift as required to provide patient care
Desired Qualifications
- One or more years of experience as a Speech Therapist
- Home health experience
- Experience working with adult and geriatric populations
- Knowledge of Medicare home health regulations and documentation requirements
