Speech Language Pathologist, SLP / ST - Home Health Visits

Job Summary
Conduct home health visits to adult and geriatric clients in Chatham, Lee, and Northeast Randolph counties to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop and revise treatment plans based on evaluations and physician orders while educating patients and families on rehabilitative care. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet as part of the multidisciplinary care team. Requires a current North Carolina Speech Language Pathologist license, ASHA certificate of clinical competence, and solid EMR skills.
Required Qualifications
- A current license as a Speech Language Pathologist in North Carolina
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
Desired Qualifications
- Solid computer skills
- prior experience with electronic medical records (EMR)
