Job Summary
Conduct comprehensive evaluations of speech, language, cognitive, and swallowing function to develop individualized plans of care. Provide therapeutic interventions using evidence-based techniques, assess home environments for adaptive equipment, and educate patients and caregivers on safety strategies. Monitor patient progress, adjust treatment plans as needed, and communicate proactively with physicians and interdisciplinary teams. Document all care accurately within the EMR and participate in case conferences and discharge planning. Requires Master's degree, current SLP license, one year clinical experience, and valid driver's license. Required Qualifications
- Master's or Doctoral degree in Speech-Language Pathology
- Current SLP license in the state of practice
- Minimum of 1 year clinical experience
- Strong clinical judgment and ability to work independently
- Excellent communication and patient education skills
- Comfortable in a field-based role with travel between patient homes
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
Desired Qualifications
- Certificate of Clinical Competence (CCC-SLP)
- Experience in home health or community-based care
- Confidence managing swallowing (dysphagia) and cognitive-communication disorders
- Ability to adapt treatment plans to real-life home environments
- A balance of clinical expertise, compassion, and accountability
