Job Summary
Conduct home health visits to adult and geriatric patients in Chatham, Lee, and Northeast Randolph counties to perform evaluations, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop and revise treatment plans based on physician orders and multidisciplinary care goals while educating families on rehabilitative care and independent living strategies. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current North Carolina Speech Language Pathologist license.
Required Qualifications
- A current license as a Speech Language Pathologist in North Carolina
Desired Qualifications
- Solid computer skills
- prior experience with electronic medical records (EMR) preferred
