Job Summary
Conduct home health visits to adult and geriatric clients in Richland and Lexington counties, South Carolina. Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop and revise treatment plans based on evaluations, physician orders, and goals while educating patients and families on rehabilitative care. Accurately document observations, interventions, and evaluations using a touch pad tablet as part of a multidisciplinary care team.
Required Qualifications
- A current license as a Speech Language Pathologist in South Carolina
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
Desired Qualifications
- Solid computer skills
- prior experience with electronic medical records (EMR)
