Job Summary
Make home visits to clients in designated geographic territories in Wilkes and Yadkin counties. Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients experiencing speech, language, communication, or swallowing difficulties. Develop treatment plans based on physician's orders and recommended goals, while continually assessing and revising care plans with multidisciplinary clinicians. Educate patients and family members in rehabilitative care to promote independent living. Accurately document observations and interventions using a touch pad tablet. Requires a current North Carolina Speech Language Pathologist license and home health experience.
Required Qualifications
- Home health experience as a Speech Language Pathologist, SLP
- A current license as a Speech Language Pathologist in North Carolina
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
- Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification
- Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification
Desired Qualifications

- Prior experience with electronic medical records (EMR)
