Job Summary
Conduct one-on-one Speech Language Pathology sessions with older adults in private residences, senior living communities, and memory care settings to support mobility, independence, and quality of life. Deliver therapy focused on wellness and functional improvement while documenting progress effectively within FOX's EMR system. Collaborate with interdisciplinary teams including PTs, OTs, and Fitness Specialists to coordinate comprehensive care plans. Incorporate telehealth and virtual care components to extend access and maintain continuity for patients. Drive to patient locations and communities as part of the Geriatric House Calls model, providing moderate to maximum assistance with transfers and bed mobility.
Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Must be able to provide moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities
- Must be available for in-home, senior living, and virtual care
- Must be located in Anderson and Fishers, IN and surrounding communities
Desired Qualifications
- Experience with telehealth and other forms of virtual care
