Job Summary
Conduct home visits to adult and geriatric clients in Independent and Assisted Living communities across Cuyahoga, Summit, and Medina counties. Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop and revise treatment plans based on physician orders and goals, while educating patients and family members on rehabilitative care. Accurately document observations and interventions using a touch pad tablet as part of a multidisciplinary care team. Requires a current state license and certificate of clinical competence from the American Speech and Hearing Association. Per diem opportunity with comprehensive benefits including paid time off, health, dental, and vision.
Required Qualifications
- A current license as a Speech Language Pathologist in STATE
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
- Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification
- Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification
Desired Qualifications
- Prior home care experience as a Speech Language Pathologist, SLP
- Solid computer skills
- Prior experience with electronic medical records (EMR)
