Job Summary
Set personalized goals and implement individualized treatment plans for patients in home health settings. Develop and execute innovative clinical and educational programs while collaborating with interdisciplinary teams to maintain productivity standards. Complete necessary documentation and serve as a trusted partner in the community to help patients achieve vital outcomes. This PRN role offers flexible scheduling with no set start or end times, including weekend availability, and provides mileage reimbursement for travel. New graduates with a CCC and state license are welcome to apply and join a mentorship program supporting emerging professionals.
Required Qualifications
- Graduate of an accredited program for Speech Language Pathologists
- CCC
- Valid state license
- Flexible availability, including weekends
