Job Summary
Evaluate residents with speech, language, cognitive, voice, and swallowing needs. Create individualized treatment plans based on clinical findings and resident goals. Provide therapy focused on communication, cognition, and dysphagia management. Track resident progress and adjust treatment approaches when appropriate. Educate residents, families, and caregivers on therapeutic strategies. Collaborate with nursing, physicians, and rehabilitation team members. Complete accurate and timely clinical documentation. Work with both short-term rehabilitation and long-term care residents in a supportive environment focused on quality outcomes.
Required Qualifications
- Active Illinois Speech-Language Pathologist license
- Strong clinical assessment skills
- Strong communication skills
- Strong organizational skills
Desired Qualifications
- Skilled nursing or post-acute care experience
