Job Summary
Provide direct rehab care in speech therapy to patients per physician referral in post-acute care settings. Responsibilities include establishing, assessing, and modifying measurable therapy goals; overseeing CFYs if SLP CCCs; following treatment plans under supervision; charting timely; utilizing therapy software; delivering excellent customer service to patients and families; collaborating within an interdisciplinary team; maintaining confidentiality and adhering to policies; and complying with regulations and Life Care standards. Must possess MS/MA in Speech-Language Pathology, ASHA CCCs or be eligible for CFY, active state licensure, CPR, and be able to work in a per-diem, on-site capacity in Carbondale, CO.
Required Qualifications
- MS or MA in Speech-Language Pathology
- ASHA CCCs OR eligibility for 9 month CFY to earn CCCs
- Active state licensure in good standing
- CPR certification (upon hire or during orientation) and maintain current CPR
- 1 year experience preferred; new grads welcome to apply
