Speech Language Pathologist (SLP)

Direct rehab care in speech therapy to patients upon physician referral; participate in community health matters; proficient in Microsoft Word, Excel, and email; liaise with patients, families, and support departments to plan for patient needs; implement and interpret department programs and procedures; oversee and evaluate care for SLP CFYs (CCC/CFY pathway); follow treatment plans for patients under supervision; chart appropriately and timely; use therapy software accurately; demonstrate excellent customer service and maintain confidentiality; participate in interdisciplinary team efforts and comply with Life Care’s Code of Conduct and mandatory training; maintain good body mechanics and attend to safety and sanitation; ability to read, write, speak, and understand English; adherence to policies including harassment and compliance procedures; ability to work in post-acute care facility setting.
