Job Summary
Establish, assess, and modify realistic, measurable, timely, and functional goals for speech therapy patients; provide direct rehab care in speech therapy to patients upon physician referral; oversee and evaluate care for CFYs when applicable; follow treatment plans under supervision; document appropriately; utilize therapy software; collaborate with an interdisciplinary team to plan patient needs; maintain professionalism, confidentiality, and compliance with company policies; support patient rights, safety, and quality of care; contribute to community health initiatives and adapt to a diverse patient population within a Life Care Center of America SNF setting.
Required Qualifications
- Graduate of an accredited program in speech therapy (minimum MS or MA SLP)
- ASHA certified (CCC) OR eligible for 9 month Clinical Fellowship Year (CFY) to earn ASHA credentialing (CCCs) and maintain credentialing
- Currently registered/licensed in applicable State with active license in good standing
- One (1) year experience preferred; new grads welcome to apply
- CPR certification upon hire or during orientation (current through employment)
- Proficiency with Microsoft Word, Excel, and email
- Ability to participate in community health matters/projects as appropriate
- Demonstrated ability to communicate effectively with patients, families, and interdisciplinary teams
- Knowledge of post-acute care facility practices, regulations, and guidelines
- Ability to implement and interpret department policies and procedures
- Commitment to maintaining confidentiality and professional ethics
- Ability to work independently and function within an interdisciplinary team
- Excellent customer service and positive attitude toward patients
- Ability to read, write, speak, and understand English
- Demonstrates good body mechanics and patient safety awareness
