Job Summary
Evaluate patients for speech and language disorders, initiate assessments upon referral, and develop individualized plans of care in collaboration with the physician and multidisciplinary team. Provide rehabilitative speech-language therapy services, carry out appropriate interventions, and educate patients, families, and caregivers on treatment procedures. Monitor, document, and report patient progress using the agency's Problem Oriented Record (POR) and required clinical documentation systems. Coordinate care with healthcare team members, consult with supervisors, and participate in case conferences. Support patients and families in adapting to limitations by providing education and resources to promote independence. Maintain compliance with agency policies, patient rights, confidentiality, infection control, safety standards, and emergency preparedness while managing workload and scheduling. PRN with flexible daytime hours.
Required Qualifications
- Active, good-standing MA Speech-Language Pathologist license
- 1-3 years of experience in geriatrics, long-term care, or short-term rehab
- Ability to communicate fluently in English - both verbally and written
