Speech Language Pathologist (SLP)

Assess referred patients requiring rehabilitation services by evaluating their medical condition, functional capabilities, limitations, and restrictions to determine potential for rehabilitation. Establish and administer treatment programs with specific goals based on patient capacity and tolerance under physician direction, while setting realistic and achievable objectives. Provide direct patient care including education on appropriate therapy methods, and document conditions, weekly progress notes, recertifications, and discharge documentation. Collaborate with all disciplines to deliver patient-centered treatment and ensure compliance with confidentiality, HIPAA, and Federal Residents Rights.
