Speech Language Pathologist (SLP)

Provide home-based speech therapy to patients with communication, swallowing, or cognitive difficulties. Conduct patient assessments, document progress, and assist with mouth, breathing, and swallowing exercises. Educate patients and families on therapy goals, safety practices, and self-care while working as part of a multidisciplinary home health team. Observe and evaluate a patient's ability to communicate and process information to ensure established goals are met. Maintain knowledge of state and federal home health regulations and exercise strong clinical judgment.
